Brian K 07:04 PM INITIAL NOTE: Admit to program 22 year old Caucasian male arrived via air from Fort Gibson, OK. He has had a problem with AOD for 6 years primarily with almost daily use of etoh. He said from the start he always would drink more than most everyone else and would not stop until it was all gone and still wanted more. He said he would usually drink to blackout. He last used etoh on 4/10/07. This is his 1st time in Tx. He has Rx lexapro that he has been on for the past 8 months for depression. he said he tried other antidepressants prior to the lexapro and this seemed to be the one that is working for him. He has Rx lexapro 10mg (133 pills) and 20mg (4 pills). His Dr has provide wriiten instructions for Brian to take 20mg twice daily. All medication is in sample form. Brian has current legal issue for 3 felonies: evading the police, running through 3 roadblocks, and a 3rd DUI. He has signed a release and will provide lawyer info to staff upon receipt from his mom. His legal Hx includes arrests for burglary, 2 DUIs, and 2 assault and battery cases. He has worked in 'dirt work' he said as a heavy equipment operator for the past 5 years in him family's business. He has made a commitment to fulfill 90 days in program, Don has been assigned as his primary counselor.

Cheryl H 05:41 PM INITIAL NOTE: Admit to the program 46 year old Caucasian female from Germantown, MD. She has had a problem with AOD for 10 to 12 years she says. She has primarily been using etoh 2 or 3 glasses of wine 3 times a week and snorting cocaine binge use for 3 days at a time and she said her usage increased over the last several years. She said she last used etoh and cocaine 2 weeks ago, approx 4/10/07. She had a heart catherization 2 months ago for potential heart attack and has been takeing Rx coreq and Rx lipitor and OTC aspirin for the past 2 months. She has Rx trazodone she has been taking for the past year for sleep disorder. She has Rx synthroid that she has been taking for the past 5 years for thyroid condition. She is allergic to epinepherine and codeine. She said she has been taking Rx klonopin daily for the past 4 years, was earlier on 6mg per day and most recently on 4mg per day, and took her last pill this am. Staff is considering sending Cheryl to a detox center for assessment to come off of this medication. She has no legal issues. She has been married for 10 years and has a grown son (23 yo). She said she once had 2 years clean 1998 and 99 but does not know how or why she did it. She said she is here because she wants to learn how to live and has made a commitment to fulfill 30 days in program. Kathie has been assigned as her primary counselor.

She has completed approx 15 days and has credit for approx 15 days red level to be used within the next 6 months. She is leaving today on the 5:20pm train out of the SJC depot. Anthony K 02:13 PM INITIAL NOTE: Admit to program 31 year old Korean male from Los Angeles, CA. He was in medical supervised detox center 4/21 to 4/23. He has been using etoh since age 15 and last used etoh on 4/20/07. he last used methamphetamine 6 years ago he said. He said that he is here mainly to address his depression and social anxiety disorder. He has had a Hx of schizo-affective disorder and depression for 10 years and had been on various medication combinations and said he has been stabilized on the medications he has been taking the past 2 years, Rx seroquel, Rx zoloft, and Rx zyprexa and says that they seem to be working for him. He has no allergies and appears to be in good physical health. He was arrested for his 3rd DUI January 07. He has been working for his family's restaraunt in downtown LA for the past 2 years. he has made a commitment to fulfill 30 days in program, Don has been assigned as his primary counselor.

Ernie T 12:29 PM INITIAL NOTE: Admit to program 41 year old Caucasian male from Dana Point, completed approx 2 days at medical supervised detox in San Clemente. He has had a problem with AOD for 20 years. He has various amounts of long term clean time (5 years, 3 years) and during the past year had 6 months but has been unable to stop using etoh on his own. He last used etoh on 4/22 and last used ativan, 2 pills, on 4/21. He has Rx antabuse 250mg (31 pills and written refill), Rx metoprolol 50mg (0 pills, 1 refill and need refill asap and written refill as well) and 1 written. Rx for neurontin 100mg, need filling asap. He has stitches in his bottom lipHe has current ongoing issue of DUI, has an attorney and signed release for letter. Written Rxs are in his medication bag and copies of Rxs in clinical file. The program director is filling the nuerontin and metoprolol Rx currently. He has mainly worked in sales type jobs and said he recently lost his manager position sales due to his etoh use. He has made a commitment to fulfill 30 days in program, Don has been assigned as his primary counselor.

He last used etoh and marijuan approx 1 week ago. He is allergic to peanuts. He will arrive to WB on 4/22. On 4/23 further assessments to be completed. Thurman has been assigned as his primary counselor.

Jane D 06:06 PM INITIAL NOTE: Admit to program 43 year old Caucasian female from Anchorage, AK. She last used etoh,cocaine, heroin, and pills on February 13, 2007. She has Rx paxil and Rx atarax. She has no allergies. Further assessments scheduled for 4/23. Kathie has been assigned as her primary counselor and she is at LW.

Grant M 03:25 PM DISCHARGE SUMMARY: Phone number 760-443-6694. Grant completed approximately 60 days at South Coast and has been clean and sober since January 21, 2007. He increased his exercise through gym membership and improved his eating and sleeping habits. He plans on annual check-ups at the VA. His plan for returning home is to live with family. He plans to return to his previous employment as an office manager, full-time, and said that this work agrees with his personality. He owns his own vehicle and he is currently on parole and plans to continue in compliance with his parole. He is divorced. He has two children, ages nine and 13 and said his family has been involved in his recovery and going to Alanon. He plans to set aside seven hours per week for family communication. He is aware of his needs and his emotional recovery and plans to continue to address these by talking with his sponsor and talking with family. He is asking for help and thanking his higher power daily and takes his own inventory. He has plans to attend church and his plan for spiritual growth includes church and meditation. He attended all meetings and activities while here at South Coast. He plans to attend 12 step meetings in San Diego county. He started classes at Saddleback in Human Services on March 19 was his first day of school. He has formulated a set of goals for himself. Immediate goals are to stay clean, go to church, finish school, play music and stay clean. The areas he will focus on the most is self reliance, self confidence and self worth. He said he's satisfied with his personal growth made here at South Coast and he said what he liked the most was the fact that his life is returned to him and he really liked his counselor. Grant should continue well if he stays clean and sober and follows his recovery plans.

Jason C 08:00 PM Jason was discharged today from Western Medical Center. he was stabilized there on Rx medications zyprexa 5mg take 1 2x a day, and depakote 500mg take 1 2x a day. He was allowed to go on pass to his mother's home in Palm Springs and will return to program on Sunday 4/22 to EM.

Toby P 07:56 PM INITIAL NOTE: Admit to the program 20 year old Hispanic male from El Dorado Hills, CA, arrived by air. This is his 4th time in Tx as an adult. He has had a problem with AOD for 7 years, primarily useing etoh and marijuana daily and occasional ecstacy pills. He last used marijuan on 3/18/07 and last used etoh on 4/11/07. He has Rx prozac ben taking the past 7 years for depression (34 pills, 2 refills) and rx seroquel taking for the past 30 days (21 pills, 1 refill) and rx prevacid for heartburn (4 pills, 1 refill). He has no allergies and appears to be in good health. He is on formal probation and is court ordered to be in a program for 90 days. His legal issues stem from burglary, felony possession, and breaking into vehicles - cases in Sacromento, and Placerville counties. he has signed a release for letter to DPO. He said he has spent approx 4 months in jail during the past 12 months. He likes to work on cars and said he likes to work with developmentally disabled children and has experience doing both. He is grateful to be here and wants to learn how to not keep going to jail by staying clean. He has made a commitment to fulfill 90 days in program, Thurman has been assigned as his primary counselor.

John K 06:18 PM INITIAL NOTE: Admit to the program 31 year Caucasian male from Frankfort, IL. He has had a problem with etoh for 5 years. He came to SCR as a result of an intervention with his family and SCR. He said he has used etoh daily for the past 5 years, 2 to 3 or more mixed drinks per day, depending on the day. He last used etoh on 4/16. He was in a medically supervised detox center 4/16 to 4/20. He has Rx clonodine .1mg (9 pills, 1 refill, Rx trazodone 50mg (11 pills, 1 refill), and a written Rx for naltrexone which may be filled if he wants to. He is in good health aside from a 29 year Hx with asthma, and has an inhaler. He has no allergies. He has no legal issues. He was a criminal law major through his baseball scholarship and started the application process with the secret service. He has worked the past 8 years in insurance, 4 of those years out of his home. He last worked November 06. He was previously engaged, relationship ended due to his unwillingness to attend counseling at that time and his etoh use contributed to that he said. He is grateful to be here and states he already is feeing much better and clear headed than he is felt in a long time. He has made a commitment to fulfill 30 days in program, Don has been assigned as his primary counselor.

David W 05:30 PM David has returned to program today 4/20 for 17 days to complete his 30 day program. He last used etoh on 4/19 and last used methamphetamine a couple of weeks ago he said. He said he is now ready to do the SCR program. He has upcoming court dates for his latest DUI hit and run on 4/4/07. Thurman has been assigned as his primary counselor, he will reside at WB.

Wensdy P 05:16 PM This staff spoke with Dr Walker this afternoon. She said that it is ok for De De to take 300mg of seroquel per night.

Kendall T 05:10 PM Received medical clearance from Dr robert Pettis in Laguna Beach both verbal and wriiten. Kendall is cleared to return to SCR with no limitations. He is still taking his course of antibiotics and may take either tylenol or ibuprofen for pain up to 600mg at at time per the Dr. He may be returning to SCR this evening and will be in the program for approx 26 days to complete 30. The program director will arrange with Kendall's mom the time of his arrival. He will reside at WB.

Ernie T 05:29 PM Met with client today. Client shared his drug/alcohol hx - and the behaviors/choices that have led him to SCR. Client remained positive - and appeared to be 'sharing honestly' throughout our discussion. Counselor validated the feelings that client shared with him - and encouraged client to utilize this opportunity at SCR, to re-engage in the 12-Step program, as he has done in the past. Conversation ended on a positive note.

William C 05:24 PM Met with client today. Client turned in all of the paperwork required to 'turn green'. Counselor praised and congratulated client for his willingness to complete these tasks. Client will turn 'green' tomorrow. Conversation ended on a positive note.

Mark I 01:25 PM Met with client yesterday. Client seemed to be in a 'good place' -0 was really pleased that other - "more mature" clients had been moved in to his house. Client had no 'major concerns' to discuss. Red Book/Denial Mngt. workbook was talked about - and the importance of getting it done. Conversation ended on a positive note.

Tia A 01:00 AM Client had a visitor for much of day. She also made a delightful Thai dish for the Westborne BBQ. She was on pass much of the day and really looks forward to tomorrows outing with Southcoast. She was bringing out clothes and asking if they'd look good to wear tomorrow. She was referring to this writer as I have given her lots of prompts and encouragement to address her negative behaviors. Some of these include leaving clothes on her bedroom floor and excessive use of phone. She has a tendency to be late when leaving the house. She is taking direction better and is less intrusive and disruptive. She is participating better in her program and getting along somewhat better with peers. Don H. R.N.

Natalie M 12:52 AM Client appeared to have a difficult, emotional day. She reports not wanting to go back with husband and having difficulty with her mother. She is unsure of where she'll live in the near future. She plans to stay in this area for an additional week and go to meetings on her own and stay connected. Mark I 12:47 AM Client expresses his determination to stay sober. He admits going to CA meetings felt a little strange because he only drank alcohol. He seems to be challenging his previous thoughts of "drug addicts", and states, "I thought of them as druggies and that's what I called them when they'd skip bail and I'd go after them." He has been employed as a type of bounty hunter for a considerable period of time. He is participating in all program activities and socializing well with peers. Don H. R.N.

John K 12:41 AM Client is attending and participating in all program activities. He works well with peers and performs additional duties (cleaning house) beyond his assigned chores. He enjoys the company of a few peers he just went through detox. with. Enjoys joking around with them and they seem to have bonded together some in a positive manner. Don H. R.N.

James G 12:37 AM Client has been somewhat withdrawn with anxious affect much of the day. He appears very preoccupied and when I approached him he admitted to feeling sad and anxious with post traumatic thoughts related to death and other horrible things he saw in war. He also misses his family a lot. Encouraged client to talk with counselor ASAP regarding his post-traumatic stress issues, and anxieties from being away from loved ones. He is friendly and cooperative upon approach and socializes some with select peers. He admits fears nobody will understand his issues and is afraid to even mention some of the things he's been through. Don H. R.N.

Hisham O 12:29 AM Client mood appears mostly calmer though he was loud and disrespectful to a staff member earlier this evening. He interrupted her while she was talking to other clients about chores and kept talking loudly about "farting" and how he had gas. Another peer was on the phone at the time within 10 feet of these offensive comments. He did participate well in house meeting and asked appropriate questions.

She was smiling and laughing a lot before his arrival and her dress and grooming were very impressive. She introduced her boyfriend to this writer and her peers. She admits to feeling so excited about the day. Client has been more social the last few days with select peers and is bonding with them. She admits to some frustration with hearing difficulty and we discussed ways to make things better including using closed captioning on the T.V. Don H. R.N.

Mark I 03:11 PM Client states, "I haven't felt this good in a while." Appears very determined to maintain sobriety and live a healthy lifestyle. Denies cravings and minimal lingering detox. symtoms. He states, "My speech is better and my shakes are gone." Very helpful during deep house cleaning today and comes to staff to get needs met. Client is very supportive of peers and considerate of others needs and feelings. Socializing well with staff and peers and a real pleasure to be around. Don H. R.N.

John K 03:05 PM Client reports daily improvement in how he feels. Slight tremors still noted but client denies any detox. symtoms. He is socializing with select peers and very pleasant upon approach. James G 02:58 PM Client somewhat isolative and withdrawn today. He reports missing his family and is having difficulty reaching his children on the phone. He has been sleeping much of the time since morning deep house cleaning. He performed these duties well and without complaint. Encouraged clients peers to socialize with client as he admits to feeling lonely now. He is friendly and insightful upon approach. Affect much brighter when socializing. Appears pre-occupied with thoughts of home and admits to feeling sad after seeing other peers with visitors. He wishes to be closer to his family and supporting them more now but aknowledges his need to be here at this time. Don H. R.N.

He is happy to be changing houses and feels this to be a progressive step for him. He has done very well while here at Big Sur and is very active in participating in his program. John K 07:39 PM Client admitted to Big Sur house this evening. Belongings checked and medications secured and logged in. Client is fully oriented, pleasant, and cooperative with admission process. He states he had a rough detox but feels much better now and denies need to fill prescription for Naltrexone at this time. Prescription placed in clients chart. He went through detox. with another peer who is also in this house. He reports being glad to be here and desire to get his life on track. Oriented to house and introduced to peers. Valuables secured and logged in. Went to evening meeting with peers tonight. D.H. R.N.

Gary V 12:25 PM Client returned to Big Sur in Van at 11:55AM and stated to this writer, "I came back to put my laundry in the dryer, get my cell phone, and deal with my jeans." This writer explained to client it was not his laundry time and not even his laundry day. It was also explained he was not to be making special trips back here when he and the rest of the clients are supposed to be somewhere else. Van driver asked me to encourage client to hurry up and when I told him they were waiting on him he exclaimed, "I'm tired of this bullshit. If they didn't bring me back I'd pack my shit and go home." He has been testing limits and very negative toward House rules. He is argumentative and when instructed on house rules/guidelines he often responds in a pressured, loud, agitated manner. He frequently attempts to persuade staff to allow him to operate outside of program rules, and will often attempt to challenge other rules when confronted with one incident. He usually gets along well with peers and his agitation appears to be directed more at complying with program and listening to staff direction. Will continue to work with this client on these above issues and praise him when he demonstrates a good attitude and improved willingness to address above issues. He is attending his groups, gym, meetings,and other program activities though frequently is the last to leave and often slows down the transportation process. Lili G 04:15 PM Lili came to the center 4/23 at approx 10am after being picked up from a 12 step peer in the community's house by staff. She tested clean for all substances. She met with the program director and the case manager and after further ivestigation it was learned that some of the previous charting may not be factual. It was decided that she return to program and will move to SM. She will remain at red level until discussion at treatment team.

Thomas D 09:25 PM At 2:45 I heard a woman yelling at the front door at El Molino. Apparently Thomas parked in front of her house, and while backing into the spot he knocked over a yellow "drive slow sign". When he exited his vehicle she ran up to him and called him some bad names and demanded he pick up the sign. Thomas said he was offended by her language and he ignored her and walked back to the house. I walked up the street with the woman, who was irate, and was able to talk her out of calling the police and eventually resolved the situation completely. We decided to move Thomas to Ocean Hills for the night in order to show the neighbors we were taking action to correct the incident. Thomas remained calm and was okay with moving, although he said he was irritated.

Chris W 09:24 PM Chris called me Saturday night to inform me he had found a room for rent and was done with South Coast. He said he would come by the house Sunday at 4pm to pick up his stuff. Chris did not show up on Sunday and I left a message on his voicemail to call Ken G ASAP.

Jason C 09:23 PM Jason came home on Saturday night. He left the house with another client at 10pm and went walking down the street. I found him and instructed him to return home and not to leave the house again without staff while he is on red level. The next morning i woke up at 8:30 to give him his meds before the client event and Jason had left the house and signed out "starbucks 9:00 am". I informed the program director and when he returned he agreed to comply from now on. When we were leaving to go to the BBQ I would not let him go back in the house to get a cup of juice and politely informed him there is no food or drink in the van. He responded by saying "you know what" and getting out of the van and slamming the door. I drove away and he called me 1 minute later and apologized in his own way. I drove back and picked him up and Jason attended the BBQ and the meeting without further event.
